This clinical guide covers the stages, technology, and efficacy of Focused Shockwave Therapy (fSWT) for Peyronie’s Disease, overseen by our specialist Men’s Health team.

Specialist Clinical Care for Penile Curvature, Pain, and Scar Tissue

Research suggests that Erectile dysfunction (ED) affects around 1 in 16 men (about 6%). However, many urologists believe the real figure is much higher, as the condition often goes unreported. In fact, it may impact as many as 1 in 10 men (10%), making erectile dysfunction far more common than many people realise.

At Bodytonic Clinic, we believe silence isn’t helpful – so let’s break it.

More importantly, let’s look at how Focused Shockwave Therapy, an advanced non-invasive treatment available at our Canada Water and Stratford clinics, is offering men real relief, renewed confidence, and the chance to take back control.

What is Peyronie’s Disease (PD)?

Peyronie’s disease (PD) is caused by the build-up of fibrous scar tissue (plaque) inside the penis. Peyronie’s disease (PD) is caused by the build-up of fibrous scar tissue (plaque) inside the penis. To understand exactly where this tissue sits within the anatomical layers, see our clinical glossary of penile structures.

This can result in:

  • Curvature during erection (often upwards or sideways) – Noticeable penile curvature during erection – often bending upwards or sideways, a common sign of Peyronie’s disease.
  • Penile pain, sometimes even when flaccid – often linked to an ‘hourglass’ deformity in Peyronie’s disease, where scar tissue affects skin retraction and causes discomfort.
  • Peyronie’s disease can also lead to shortening or narrowing of the penis. This is sometimes referred to as an ‘hourglass’ deformity, where scar tissue causes restriction and changes in shape.
  • Erectile dysfunction in many cases.

It’s more common than most realise. Studies suggest between 3% and 10% of men in the UK are affected, though underreporting means the true number may be higher. The condition usually develops in men over 40, but it can occur at any age.

Risk factors include previous penile injury, diabetes, hypertension, and conditions like Dupuytren’s contracture.

  • Why Peyronie’s Disease Matters
  • Beyond the physical symptoms, Peyronie’s can take a heavy toll emotionally:
  • Strained relationships
  • Anxiety and depression
  • Loss of sexual confidence

Research shows up to 70% of men with Peyronie’s also experience erectile dysfunction. So, it’s not just a physical curve – it’s an emotional U-turn too.

🔬 Peyronie’s Disease Stages

Peyronie’s Disease treatment depends heavily on the stage of the condition. We differentiate our approach based on these two phases:

  1. The Acute Phase (Early Stage): This phase typically lasts 6 to 18 months. Symptoms include pain during erection, and the penile curvature may still be changing. The goal of treatment here is primarily pain reduction and minimising plaque development. Focused Shockwave Therapy (fSWT) is highly effective during the acute phase for providing significant pain relief and stabilising the condition.
  2. The Chronic Phase (Stable Stage): The plaque is fully formed, the curvature is stable, and pain has usually resolved. The primary concerns are penile shortening, curvature, and associated erectile dysfunction (ED). The goal of treatment is to remodel the plaque and restore function. In this phase, fSWT works to break down or remodel the fibrous plaque and stimulate blood flow to improve erectile function.

The goal of treatment is to remodel the plaque and restore function. You can learn more about the internal columns and tissues involved in our MSK ED & Men’s Health Glossary.

Traditional Treatments vs. Modern Approaches

Before considering surgery (which is typically reserved for severe, stable cases), men often explore these non-invasive and minimally invasive treatments:

  • Oral Medications: Drugs like Pentoxifylline, L-Carnitine, and Vitamin E are often prescribed by GPs or urologists. However, clinical evidence for their long-term effectiveness in reducing plaque size or curvature is very limited and results are inconsistent.
  • Injectable Therapies (e.g., Collagenase/Xiaflex): This treatment involves injecting an enzyme directly into the plaque to break down the collagen. While effective for some, it can be costly, require multiple office visits, and availability and suitability for this treatment vary significantly in the UK.
  • Penile Traction and Vacuum Devices: These devices are used to stretch the penis and may help reduce shortening and curvature. They are often prescribed to be used in conjunction with injectable or Focused Shockwave Therapy to maximise results.
  • Surgery: This is typically reserved for severe, stable cases where curvature is extreme. While effective, it is invasive and carries risks, including penile shortening and sensory changes.

Why Focused Shockwave Therapy? fSWT is a non-invasive, drug-free alternative that can be used in both the acute and chronic phases to provide pain relief and improve the underlying vascular health, without the risks associated with needles or major surgery.

These approaches left many men with “wait and see” advice. But modern technology now offers something more targeted.

Focused Shockwave Therapy – The Bodytonic Difference

At Bodytonic Clinic, we use the Venn Healthcare focused shockwave system – a medical-grade device designed to deliver precise, deep-penetrating acoustic energy into scar tissue.

How It Works

  • Targeted energy: Focused shockwaves break down fibrous plaques.
  • Blood flow boost: Stimulates new blood vessel growth (neovascularisation) to support tissue recovery. A full explanation of these vascular health and healing terms can be found in our comprehensive glossary.
  • Healing activation: Encourages the body to remodel scar tissue.

Clinical Note: Not all shockwave is the same. For Peyronie’s, the depth of the plaque requires a convergent focal point. To understand the physics of how our Piezoelectric system differs from standard radial devices, read our Full Clinical Guide to Focused vs. Radial Shockwave Therapy.

Benefits

  • Significant pain reduction
  • Softening and remodelling of plaque
  • Support for erectile function
  • Non-surgical, quick, discreet sessions (20-30 minutes)
  • No downtime – perfect for busy London schedules

Unlike radial shockwave (used for muscle injuries), focused shockwave penetrates deeper and delivers energy precisely where it’s needed.

What the Research Shows

The science behind focused shockwave therapy for Peyronie’s is evolving, but results are promising:

  • Pain relief: Long-term studies show up to 67% of men report significant pain reduction.
  • Erectile function: Around 60% see improvement in erections and sexual performance.
  • Patient satisfaction: A 10-year follow-up study found 90% were satisfied with shockwave treatment.
  • Curvature changes: Some men experience measurable straightening (in 20-70% of cases), though results vary.

Guidelines from urological associations highlight shockwave’s strong role in pain relief, with growing evidence for wider benefits. While not a miracle “penis straightener,” it is a powerful, safe option – especially when combined with other approaches like traction therapy or PRP (P-Shot).
