Lumbar Disc (Disc Herniation): Causes, Symptoms & Expert Treatments

What is Lumbar Disc (Disc Herniation)?
The spinal discs (intervertebral discs) are located between each vertebra from the cervical spine down to the lumbar region. They function as essential shock absorbers and tough connective tissue, holding the vertebrae together while providing joint stability and allowing for the spine’s range of movement.
Lumbar discs are specifically designed to manage the significant pressure and physical loads of daily activity. In our youth, these discs have high water content. However, through the natural ageing process or repetitive strain, discs can lose fluid and become compressed. When over-pressured, the firm outer layer may irritate, strain, or allow the inner “jelly-like” content to push out of place. This is medically known as Disc Herniation (often colloquially called a “slipped disc“).
What are the signs & symptoms of a Lumbar Disc (Disc Herniation)?
Signs and symptoms that may happen with a lumbar herniated disc include:
- Lower back pain: Persistent aching or sharp pain in the lumbar region.
- Sciatica: A general term referring to nerve irritation. In this case, it means an affected disc is pressing on a nerve, causing pain.
- Radiating Pain: Pain that travels from the lower back or buttock down the leg, normally feeling like a shooting or electric sensation.
- Postural Aggravation: Pain aggravated after prolonged standing or sitting.
- Numbness or Pins & Needles: Tingling sensations in the affected leg caused by nerve root compression.

- Muscle Weakness: Weakness in the leg or foot due to the muscles being served by the affected nerve; this may cause you to stumble.
- Cauda Equina Warning: Some people can lose bowel or bladder control due to cauda equina. This is an extremely rare condition where multiple lumbar and sacral nerve roots are compressed.
Rare but Urgent: If you experience a loss of bowel or bladder control, or numbness in the “saddle area,” seek emergency NHS medical attention immediately. This may indicate Cauda Equina Syndrome, a rare but serious condition.
What are the common causes of a Lumbar Disc (Disc Herniation)?
Most lumbar disc herniations are of gradual onset. Disc wear and tear is a common condition as people grow older. While the firm outer layer of the disc dehydrates over time, it may develop a crack. When the disc becomes less flexible, the inner jelly-like layer is more prone to push through the crack with even a minor twist or load.
Besides gradual onset, disc herniation can result from a traumatic event such as lifting a large and heavy object, a fall, or a direct blow to the back.
Risk Factors:
- Excess Body Weight: Increases pressure in the lower back, causing extra stress to accumulate in the lumbar spine.
- Repetitive Movements: Pulling, lifting, bending, or twisting are demanding for lower back joints. Your occupation significantly impacts back health.
- Genetics: Disc herniation can be hereditary; a family history increases your risk.
- Smoking: Decreases oxygen supply to the disc, causing it to dehydrate and damage more quickly.
- Driving: Long-duration sitting plus car engine vibration increases pressure in the lower back.
- Sedentary Lifestyle: Leads to general weakness and poor support in the lower back area.
How is Lumbar Disc (Disc Herniation) diagnosed?
To understand if you have a herniated disc, you should see a medical professional or a GP. A herniated disc is suspected if you have lower back pain with radiated leg symptoms.
Clinical Assessment:
A neurological examination will be completed by our medical professionals. The exam tests muscle strength, sensation, reflexes, and upper motor neuron signs. Specific tests, such as Straight Leg Raises, are helpful to indicate nerve irritation.
Diagnostic Imaging:
- X-ray: Shows bone structure but not soft tissue.
- MRI (Magnetic Resonance Imaging): The best imaging study to support a diagnosis, as it shows organs and soft tissues, including intervertebral discs.
- Myelogram: Uses contrast dye to look for problems in the spinal canal.
- CT Scan (Computed Tomography): Uses X-rays and computer technology to show bones, fat, and organs.
- EMG (Electromyography): Measures muscle responses and electrical activities via nerve stimulation.
What are the treatment options and how long does it last?
For the majority of cases, a herniated lumbar disc will gradually improve over 4 to 8 weeks. Most patients are pain-free after 4 months. Initial treatments focus on non-surgical, conservative care.
Initial Management & Rest:
Resting at the first onset usually helps relieve symptoms. However, you should resume activities as pain tolerates to avoid muscle loss.
- Spread out daily tasks and rest in between.
- Prevent sitting for long periods.
- Keep physical activities low-intensity and controlled.
- Avoid aggravating factors.

- NSAIDs: Anti-inflammatory medication can help with pain control.
- Steroid Injections: An effective way to reduce inflammation around the nerve, providing short-term relief (beneficial if symptoms last >6 weeks).
- Surgical Treatment: Advisable only if conservative treatments fail. Microdiscectomy is the common procedure to remove the herniated part compressing the nerve.
